Insurance

We are in-network with Medicare and Tricare.  Please obtain a referral/prescription for occupational therapy from your physician/NP or PA.  Tricare Prime beneficiaries need to obtain prior authorization through their base clinic/Tricare prior to making an appointment.

Rise Pelvic Therapy & Wellness is not contracted with any commercial insurance plans and service is provided on a self-pay basis.  Full payment is due on the day of service.   Upon request, we will email you a superbill that can be submitted to insurance for possible reimbursement using your out of network benefits.  Seeking reimbursement is the sole responsibility of the client, reimbursement is not guaranteed and Rise Pelvic Therapy is not able to assist in seeking reimbursement.

Patients with Medicare Advantage HMO plans do not have out of network benefits and will be accepted only on a self-pay basis.

Initial Evaluation   $200

Follow-Up Appointments  $175

Payment Policy

Cash, credit card (including HSA/FSA) and check are accepted for your session and full payment is due at the time of service.  Returned checks will be assessed a $50 fee.  

Cancellation Policy

We have a 24-hour cancellation policy.  Making your healthcare and recovery a priority allows for the greatest outcomes possible for you.  We also want to provide time for our other clients to be notified of appointment availability.  There is a $60 fee charged for appointments cancelled within 24 hours of your appointment time.  Cancellation charges must be paid prior to scheduling a new appointment.   Please contact us ASAP if circumstances arise beyond your control.  We reserve the right to discharge care if more than two appointments are cancelled without adequate notice.
